FMS Extended Day Enrollment Form

If you plan to utilize FMS Extended Day, please fill out the attached form on the link below ASAP. Payment is due on Monday, August 11, prior to starting but with it being the beginning of school, we will take payments until Friday, August 15.  If the payment is not made by then, your student will not be able to participate in the program until payment is completed.
